CASE STUDY
Dr Tania was elected the Director-General of the International Aid and Health System in 2006. Born in Europe Dr Tania obtained her medical degree from the University of Western Ontario in Canada and then commenced her distinguished career in public health in Taiwan in 1980. In her career of more than 40-years Dr Tania has been required to lead various local and global aid and health initiatives. The major events which highlight the career of Dr Tania include the 1996 Nipah virus epidemic; 2005 avian flu outbreak; 2015 polio in Afghanistan; 2019 Ebola outbreak in central Africa.
Having commenced and progressed through her career rapidly, Dr Tania was appointed Chief Medical Officer for Taiwan in 1993. In the first case of dealing with a national health crisis, Dr Tania was responsible for leading the national response to the Nipah virus. Transmission is thought to have occurred via unprotected exposure to secretions from the fruit bats, or unprotected contact with the tissue of a sick animal. In subsequent outbreaks consumption of fruits or fruit products (such as raw date palm juice) contaminated with urine or saliva from infected fruit bats were the most likely source of infection. It was not until more than 8% of the population were infected and there was a public outcry that the crisis was recognised by the health system. In response to the rapidly increasing number of cases, Dr Tania demanded increased case management from the hospitals and emergency medical systems. At the peak of the epidemic, the hospitals and medical systems were beyond capacity and in need of emergency funding, and Dr Tania was recognised for the efforts in lobbying the Government for funds to manage the new cases. An outcome of the limited action by the Health Department was for all non-essential medical procedures to be postponed in preference for the management of the epidemic. As a means to calm public hysteria and fear, Dr Tania embarked upon a campaign of publicly eating fresh fruit and releasing media statements suppressing the real state of the epidemic. Although there were the efforts at presenting a public image by Dr Tania, there was still a need to bring a collective approach to the epidemic. Dr Tania identified a range of stakeholders and formed a loose knit working committee to seek a range of individual solutions or activities for each stakeholder. As interim measures Dr Tania demanded that the farming industry immediately take action to remove or eliminate bats from the orchards etc.; processors/packagers increase the washing and preparation of the produce and resellers provide sampling and guarantees of food safety. The result of the working committee was that the various stakeholders did not comprehensively integrate their activities to an integrated working plan or common set of goals. Although the economic and social impacts were significant, there remained a reticence to proactively seek the support of a broad range of stakeholders and participants. Ultimately because of the rapid decrease in fruit and vegetable consumption and improved quality management by the producers the numbers cases declined. After a period of more than nine months the health system was able to manage the numbers of new cases along with existing medical procedures and elective medical procedures resumed.
Following her extended career as Chief Medical Officer for Taiwan, Dr Tania was appoint the General Director of the International Aid and Health System (IAHS). In this position Dr Tania, was responsible for managing and developing the international health systems to prevent the outbreak of disease and bring equality to the national health systems around the world. Headquartered in Geneva Suisse, IAHS was recognised for the quality of its humanitarian work and was well supported financially and politically by the United Nations, International Monetary Fund, European Union and United States. The major and most defining event in Dr Tania’s career was the Ebola crisis in central Africa in 2019. In this event the Ebola outbreak commenced in Niger but quickly spread to Chad, Democratic Republic of Congo, Cameroon and Nigeria.
From the first outbreaks of Ebola in western Africa there were many lessons learned regarding the containment elimination of an outbreak, but conclusions had considered the transnational transmission of Ebola. For the IAHS and Dr Tania the 2019 Ebola event was marked by a range of immediate challenges including: national rivalries between the five countries which had previously engaged in military combat or retained a heavy military presence along the borders; diversity in the economic and health systems, (basic through to advanced); disparate levels of education; and diverse standards of living from poverty to middle class. Dr Tania knew that these and numerous other factors prevented the control and eradication of the disease. Alone in her office in Geneva, Dr Tania commenced planning IAHS’s approach.
