During your afternoon shift at 5pm, a 66-year- old lady, Doris Evans, is brought into ED by her husband. DOB: 12/11/1955. 
Initial Assessment 
Doris is short of breath, pale and cyanosed. She is speaking to you in short sentences while holding her chest. She is coughing most of the time. 
SAMPLE: Signs & Symptoms/ Chief complaint 

• Doris is complaining of a sharp/ stabbing chest pain, about 6/10, dyspnoea (difficulty breathing) and a productive cough. General muscle pain. These symptoms started about two to three days ago. She has been taking Paracetamol for the pain. She thought the pain was going to ease until yesterday when she started coughing producing yellowish sputum. 
• Allergies
• Allergy to Penicillin – Rash reaction Medications 
• Telmisartan 40mg orally daily Rosuvastatin 10mg orally nocte Temazepam 10mg orally nocte 
• Past Medical and Surgical History 
• Hypertension 
• Hypercholesterolaemia 
• Insomnia 
• Doris stopped smoking 2 years ago. Has been a smoker since her teenage 
• years. 
• No-one has had COVID19 in the family. 
• Positive Family History of heart disease – Father died aged 52 
• Hysterectomy four years ago 
• Last oral intake
