This assignment allows for the evaluation of your understanding of the process of managing risks associated with elevated work consistent with the course materials. The assignment is graded out of 20 and is worth 20% of your final course mark. The assignment has been designed to assess your knowledge in planning work at height and your skill in communicating the risks to various levels of the organization. It also requires you to further develop your thinking and knowledge of hazard identification, risk assessment, and the control process you may have used in your experience as a safety practitioner for general tasks. You will be graded in part based on your alignment in approach to fall risk management, and your thinking about the fall protection as discussed in the course materials.

The assignment requires that you to complete a fall risk assessment, a written fall protection plan, a narrative for communicating the plan, and a written job procedure suitable for completing work at elevation. You can use your own work location, another location with permission, or a fictitious location. Describe the nature of the business and the work site(s), identify the number of employees at the site(s), and describe the general layout of the operations.

The assignment requires you complete original work created for this course assignment. Please do not submit corporate documents related to fall hazard assessment or safe work procedures developed previously or by others.

The time required to satisfactorily complete the assignment will typically be about 8-10 hours for research, organization, writing, and editing. The three parts of the assignment are fully described below. You must complete all three parts as they relate to the business and work site(s) you have chosen.

Part

1. Fall Risk Assessment

Provide a detailed fall risk assessment spreadsheet/table for at least five routine jobs performed at elevations. Also, provide the risk ranking scheme used to quantify the risk. Two marks will be given for the assessment of each job. This is intended to be presented to management to help them decide whether to implement further controls. The risks should be assessed considering the current work practices of your company and its employees. For each job, the spreadsheet/table should contain, at least, the following columns:

III. Description (identify the mechanism of potential fall/incident)

IV. Probability

V. Severity

VI. Risk

VII. Rationale (rationalize the risk variables chosen - observable)

VIII. Action/Comment (justify the actions suggested - presumable)

2. Fall Protection Plan and Narrative for Communicating the Plan

a) Provide a written fall protection plan for two non-routine jobs/tasks. Prepare a standardized form and fill in the necessary details. Try to keep it to one or two pages. Part a) is worth

b) Provide a narrative (or outline) of how a supervisor would communicate the plan with employees performing the work. Part b) is worth it.

3. Written Job Procedure for Completing Work at Elevation

Provide a complete written procedure for a routine job performed at an elevation. The procedure should contain all the information the worker needs to perform the job, such as: What are the hazards? Why is it important? What tools and equipment are required? Who is responsible for what? What steps must be taken? etc. A complete response will have 5 items for Part 1, 2 items for Part 2a), 1 item for Part 2b), and 1 item for Part 3. If you number the parts of your submission this way it will be easier to see that you have submitted everything you need. The section outline could look like this.
