Based on content from your textbook A Guideline on Protecting the Health and Lives of Patients in Hospitals", create a hospital log book for someone you know well (a friend, family member, yourself, etc.) that would accompany them during a hospitalization. The requirements of such a book are described in various parts throughout the textbook.

Explanation of the book for users. This section must minimally include- a description of need for medical safeguarding, key definitions, responsibilities of the advocate/protector, and how the book is to be used

Incorporates multiple categories of key information about the person, as detailed in the book

Important information sheets to be used by advocates, as described in the book
