Task 1: Prepare a Hazard Identification, Risk Assessment and Control (HIRAC) document for the work described below.
Field trip details:
You are a PhD student involved in a project that requires you to drive a university vehicle to a remote forest site to collect Scat samples of Koalas, and to collect images from camera traps you have set. The remoteness of the site means that there will be poor mobile coverage and the Australian bush has many poisonous snakes and spiders, fallen or falling branches and it is easy to be injured or become lost. Once collected samples and images are to be returned to the lab for later analysis (not included in this HIRAC).
Task 2: Hazard & Incident Investigation (5 marks)
As the safety officer conduct an investigation into an incident, reported to you following the events. Read the version of events written by the PhD student (Harriet Jones) involved and conduct an incident report.
-Fill out “Part 2” of the Hazard/Near Miss Report below itemising up to five (5) areas of OHS practice that need improvement, what corrective actions need to be taken to improve and who is responsible for implementation of improvements to prevent this occurring again

Description of Hazard/Near-Miss:I set out to the site before dawn and decided to tell my supervisor later when I got to the National park so as not to wake her too early. I packed lunch, water, took my mobile and GPS. I drove to the national park (about 2hrs) and listened to the radio and heard that there was a total fire ban and a high fire risk day, but I don’t need to use tools or light a fire so it doesn’t really affect me. I drove up the dirt road into the park and as I got further from the highway radio reception became poor and I lost the signal. I was searching for another radio channel when a wombat ran across the road in front of me. I swerved slightly and slammed on the brakes but the car skidded off the track and hit a tree. The airbag went off and I was winded and shaken but not really hurt I had my seatbelt on. Everything in the vehicle slammed forward and crashed off the inside of the car and the GPS broke.inclusion in the incident investigation.
