Introduction

Physical and cognitive development during childhood and adolescence depend on a healthy diet and nutrition (1,2). Nutritional guideline recommends that children and young people to consume protein rich foods, varieties of fruits and vegetables, grains and dairy products and to limit the consumption of added sugar, saturated and trans fat and sodium (3-6). However, nutrition surveys reveals the prevalence of lower consumption of fruit and vegetable and other significant nutrients, and higher consumption of energy-dense nutrient poor (EDNP) food items among all age groups (3-10). However, it is evident in the current literatures that a trend towards poor eating behaviors, including high consumption of sugary drinks, breakfast skipping tendency, and low consumption of fruit and vegetables are mostly prevalent among young individuals (11-13). Less healthy dietary behaviour is an important factor underlying the high prevalence of chronic non-communicable diseases, including cardiovascular disease and diabetes mellitus, and increasing obesity rates (14-16). Early signs of these chronic diseases and risk factors are increasingly being identified in children and adolescents (17,18). Healthy dietary habit and nutritional status are therefore necessary to maintain healthy growth and development at an early stage of life and to prevent the development of juvenile chronic illnesses (19).

To provide interventions among large number of children and adolescents from different culture and family background, school setting is the most convenient medium of interactive environment due to its existing structure, curricula, policies (20). School-led interventions help adolescents, as well as their families and broader communities through health promotion (21). School-based intervention programs are not only structured to involve students as participants but are set of strategic programs that include courses, videos lessons, lectures, poster and powerpoint slide presentations, debates and workshops, training attended in schools that include students only or sometimes involve parents, teachers and staff as well (21). School-based interventions include several types of components implemented to alter unhealthy and risky behavior of students (21). Furthermore, schools are strategic outlets for providing preventive health services which are collectively considered to be an expanded primary health care wing (22).

Considering the large numbers of reviews and research syntheses available with regard to the effectiveness of school-based interventions implemented to address dietary behaviour and

nutritional aspects, an overview of the published reviews could be beneficial to elaborate to its potency and deficiency. Overview of reviews are currently performed to critically evaluate and appraise the published reviews and to scrutinise the strength, weakness and possible improvement on a whole of a certain subject (23). The wider spectrum derived from an Umbrella Review is also essential for highlighting the coherence or contradiction or inconsistency of the evidence of school-based interventions aiming at healthy dietary pattern and nutritional balance among children and adolescents (24). The study of evidence in the Umbrella Review enables evaluation and assessment of whether reviewers investigating similar research questions independently acknowledge the corresponding outcome or postulate further concluding remarks (24). Hence, this umbrella review will allow us evaluating the constructiveness of the school-based interventions that are already published. We will undertake Joanna Briggs Institute (JBI) umbrella review method to systematically conduct this review. JBI Umbrella Reviews authorises the inclusion of all kinds of research synthesis, such as- systematic reviews (meta-analysis and/or narrative), narrative reviews, scoping reviews, critical reviews, rapid reviews etc (24). Therefore, the inclusion criteria of this umbrella review will not be restricted to a certain kind of review synthesis. Considering all kinds of review articles published on school-based interventions targeting healthy diet and nutrition will enrich the outcome of this umbrella review with comprehensive exploration.

Objectives

The following question will lead this umbrella review:

Are the school-based interventions targeting healthy diet and nutrition effective, feasible and acceptable?

To successfully answer this question, we will explore the following objectives:

  • To evaluate the effectiveness of school-based interventions on healthy diets and nutrition using any standardised measure
  • To evaluate the feasibility and acceptability of these interventions
