Your SMART goal is your title - a single sentence. For example: Reduce catheter associated urinary tract infections by 90% (from 10 infections to 1) by March 2021.
Introduction - Explain what you want to do and provide rationale for the work. The paragraph might start with the goal and then explain impact for the patient, resources and costs.
Body - finalizing the analysis bringing it all together. This means integrate all findings here, not separately with each type of analysis, as you will have done in Assessment 2. Include any any relevant literature, any relevant local data (deidentified local data is fine) that supports your discussion. Make at least 3 recommendations. The recommendations will be clearly supported by the analysis findings. Remember to include any risks and risk mitigation strategies as appropriate.
Conclusion - Summing up here of what you have done and what has been achieved. Consider how you will evaluate or sustain the solutions or recommendations. Close with where to from here or next steps in this work.
