Task:
Students in groups of 4-5 will simulate the role of primary care nurses to coordinate care that promotes healthy ageing for an authentic case of an older person. Student/s (1 or 2 only) are to have a conversation with an older relative or person in their community and gain consent to present their story to the class for the purpose of this assessment.
Part A: Students will then work as a group to deliver a comprehensive plan that supports the older person’s continued wellbeing in their community setting. Students will present their findings to the class in workshop three as a group presentation.
