Introduction
Falls are commonly reported incidents in Australian hospitals with 1 in 3 of all patients experiencing a fall (REf). Consistent with international data reported fall rates and are higher among older patients. While the majority of falls result in no or minor injury; one-third result in moderate injury and 4%–6% result in serious injury (Ash et al., 2016; Morse et al., 2018). Fall-related injuries increase the length of stay in hospital and can incur substantial costs (Bates et al., 2015). It is widely acknowledged that a significant proportion of falls in hospitals are preventable (Cameron et al., 2017), making falls prevention an important patient safety issue. Best practice in fall and injury prevention and management requires health service organizations to establish and maintain systems for the prevention of falls and harm from falls (ref- national standards). This includes prevention strategies as part of standard care, screening and assessing patients for falls risk, and implementing targeted multifactorial falls prevention strategies that are resourced adequately, monitored and reviewed regularly.
Aim
The aim of this work-based project is to achieve 100% compliance with local policies and guidelines regarding the completion of fall risk assessments for all patients admitted to the ward.
Objectives:
• To examine the impact of falls quality improvement package on the timely and accurate completion of Ontario and FRAMP as per local policies and guidelines and fall incidents by conducting a pre and post-test project
PROJECT DESIGN AND METHODS
Project setting: The project will be conducted in a 30-bed medical ward at a principal referral and teaching hospital in Sydney with approximately 800 beds and 500 full-time equivalent staff. The hospital serves a population of approximately 350,000 and provides care for over 50,000 patients per year. Project design: A pre-test/post-test design (reference required) will be used to examine the impact of falls quality improvement package on fall incidents and the timely completion of Ontario and FRAMP as per local policies and guidelines. Appendix A provided an overview of the proposed timeframes for the project.
