• Atrial fibrillation-diagnosed 6 weeks ago. Discharged on Bisoprolol (currently 5mg daily) and apixaban 5mg BD. Echo during this admission stroke volume 35%. 
 

History of presenting illness 
An ambulance was called to Olga's home this morning as she had activated the Medical alert system. On arrival the paramedics found her sitting in an armchair and looking pale. She was alert and oriented to time place and person. Her BP was 90-60, HR 125 and irregular (monitor showed AF) RR 23Histh, 5p02 96% on room air. Olga also complained of right hip pain but indicated she had not fallen today or recently. Olga said she had pushed her buzzer because she was feeling dizzy and nauseous this morning and had needed to sit down quickly. She said she felt that her heart was racing like it had about a month ago. She said she did not feel better after 30 minutes so she pushed her buzzer. Olga was brought to hospital for review of uncontrolled atrial fibrillation.
