? Organise the analysed information in a hierarchical order and outline the interacting relationships between each component/layer.

The purpose of the concept map is to gain an in-depth insight of a person living with a chronic illness.

2) Use the map to connect concepts and facts (nodes) and to demonstrate relationships between concepts such as: determinants of health; the impact of living with a chronic condition; individual personal likes, dislikes, aspirations and dreams; spirituality or cultural needs; and relationships. Links between concepts/facts need to be labelled with descriptive words or short phrases representing the relationship and providing direction to the concept.
