What is this task about?

This task is an individual task and requires you to develop a handover assessment report, for a client named called Matteo (AKA “Matt”) G. Matt is a 60-year-old gentleman who has developed aphasia following a stroke. You will be required to analyse and interpret provided referral information, assessment data, and make observations from a short video recording of Matt, to develop a handover assessment report to a communitybased speech pathologist. You will write your report for a speech pathology audience using a template. This report template is available on LEO and must be used.

What is the context of this task?

As a student speech pathologist, you are completing your professional practice placement at the rehabilitation hospital where Matteo “Matt” has been receiving inpatient rehabilitation on the background of a L) MCA stroke following cardiac surgery.

Matt spent 6 days on the acute ward. On day 7 post-stroke, he was transferred to the inpatient rehabilitation unit where he has been receiving physiotherapy, occupational therapy, and speech pathology input. He has been on the rehabilitation unit for 4 days and will stay for a total of two weeks. Following his inpatient rehabilitation, Matt will be discharged home. Matt will require further speech pathology services, which he will receive as a community outpatient.

During his inpatient rehabilitation stay so far, Matt has completed the Comprehensive Aphasia Test (CAT) (Swinburn, Porter & Howard, 2006). He received some initial intervention to enhance his functional communication (i.e., use of gesture and writing to compliment his verbal attempts). The multidisciplinary team (MDT) and his family have asked for communication strategies to engage Matt successfully in conversation and therapy tasks.

Your professional practice educator (PPE) has asked you to write a speech pathology assessment report that will be forwarded to the outpatient speech pathologist who will be providing ongoing outpatient intervention once Matt is discharged home. Your PPE has asked you to integrate the referral information with the CAT results (which your PPE has provided you with). As Matt’s inpatient rehabilitation stay will be brief, your PPE also asks you to include:
