Question

1. What is this assessment about?

2. What am I being asked to do with the anomia therapy I have been allocated?

The case study of Margaret

  • Margaret is a 60-year-old female who is experiencing progressive anomia due to a dementia not yet specified. Margaret was born in the United Kingdom and moved to Australia as a young girl. Prior to the onset of his dementia, she was fluent in both English and Japanese. Margaret converses in Japanese via weekly video-chats with her host family and friends she made when she taught English in Osaka, Japan (when aged 28-35). Margaret speaks only English with her husband, Jim, her elderly Mother, and friends. Margaret is continuing to undergo neuropsychological testing to confirm the type of dementia she has. She was referred to speech pathology services by her Neurologist after several months of increasing difficulties recalling names of people and places, making errors with appointments (Margaret owns a small tutoring business for secondary school students), and telling the time.
  • Margaret’s husband also reports that she is experiencing some difficulties with attention. He has noticed that Margaret can become impatient after a short period, posing a risk to her safety at home and in the community (e.g., climbing ladders to rescue possums at night and arguing in shops about being overcharged when she has not or accusing people of pushing in line at the post office, for instance). Margaret has a general awareness of her difficulties, telling her husband “I know there is something wrong with my brain”.
  • Margaret lives at home with her husband, Jim. They have been married for 19 years and have no children together. Margaret’s procedural memory remains intact and she continues to enjoy bike riding on local bike paths, playing the piano, observing and feeding the local ringtail possums in their garden, birdwatching when she and Jim go on day trips to the country, and practising speaking Japanese (although Jim reported Margaret is also experiencing word finding difficulties in Japanese as well as English). Margaret visits her Mother, who is in a residential aged care facility 2-3 times per week, and reads “the classics” to her Mother, who has macular degeneration. Margaret has decided to sell her student tutoring business and retire as “it’s all too much”. Margaret reported that Jim is managing the sale of the business.
