Personalised care as identified in the NHS (National Health Service) Long Term Plan (2019) promotes the nurse to consider the ‘person’ as opposed to the ‘patient’ (NHS England, 2019; Nursing and Midwifery Council [NMC], 2018). In doing so, you will need to use appropriate communication skills and interpersonal skills to support and enable the person to make informed choices within their episodes of care. This assessment will centre around a person’s story and episode of care (episode of care = services provided to a patient within a specific period across a continuum of care in an integrated system). 2 Assessment Information/Brief – Revised 12.10.22

The expectation is to choose a person’s episode of care from your practice experience and address the assessment criteria set out under ‘Assessment Criteria.’ If you have not attended placement for whatever reason, you can choose an episode of care from the patient stories below. Please only use an online patient story if you have NOT attended placement.

Assessment Criteria

  • Write a clear introduction for the assignment 
  • Briefly summarise a person’s episode of care 
  • Describe the six components of ‘The Personalised care operating model’ as per the NHS long term plan (2019), in relation to the episode of care and explain the importance of partnership working between the nurse and person in achieving a joined-up approach to their needs
  • Explain how a nurse can use communication and interpersonal skills within the episode of care, identifying what matters to the person
  • Describe one advantage of and one barrier to interprofessional teamwork in relation to the persons episode of care
  • Describe how social prescribing could support the persons health and wellbeing as well as the health and wellbeing of their family and carers. 
  • Summarise the key points you have discussed in a conclusion 
  • Provide a full reference list for the sources of literature you have used following the current. Please note: Final reference list is NOT included in your word count
  • Use a range of credible literature to support your discussion
