Brodie is a 25-year-old male from Melbourne who works in the retail sector. During midsummer Brodie and a few friends travelled to Northern Queensland for a two-week holiday. Brodie loves the outdoors and is a keen bushwalker. It was a particularly wet season that year, but that did not deter Brodie from bushwalking throughout the trip. He was not put off by the rain, humidity, and mud; the tropical conditions just added to the sense of adventure. Although thank goodness for insect repellent as there were a few mosquitoes around! Brodie and his friends also spent time by the water exploring rock pools at low tide. About half-way through the trip Brodie did graze his ankle on one of the rocks by the rockpool but it was minor – not too much blood or skin lost. Most nights Brodie and his friends partied. It would be fair to say that Brodie engaged in excessive alcohol consumption by binge drinking most nights. After a fantastic two weeks away, Brodie returned to Melbourne. One week after arriving back in Melbourne, Brodie was feeling unwell and made an appointment with his local General Practitioner (GP). Brodie had a cough and was experiencing some chest pains. He also had a fever. After examining Brodie, the GP diagnosed pneumonia and sent Brodie home with a prescription for a five-day course of the antibiotic penicillin.Brodie took the antibiotic as prescribed, but he did not improve. In fact, his condition had deteriorated. Three days after first seeing the GP, Brodie returned for a second appointment. He had increasingly severe chest pain, a cough, fever, headache, shortness of breath and, because he felt so unwell, Brodie had also lost his appetite. Upon examination the doctor couldhear abnormal chest sounds, and it was clear that Brodie was extremely unwell and required urgent medical attention. Upon presentation at hospital the GP’s notes revealed that Brodie wasan otherwise well young male with no significant underlying medical conditions. Brodie’s recent travel history and activities were documented, the presentation and development of Brodie’s symptoms were included as well as an additional statement that said, “community acquired pneumonia, not getting better on penicillin, penicillin resistant Pneumococcus?” Brodie was sent for a chest X-ray which revealed significant left lung consolidation. A specimen was collected from Brodie and, using appropriate diagnostic techniques, the etiologic agent responsible for Brodie’s infectious disease was determined
