Answer 1
The drug Metoprolol is an antihypertensive and belongs to the class of beta-blocker drugs used to manage hypertension. David has been prescribed metoprolol to
normalise his blood pressure. The mechanism of action can be explained by the fact that being a beta-blocker, it acts on the beta cells in cardiac muscles and reduces the
Na2 ion reuptake and also confines the outflow of K+ ions, which reduces cardiac contractility and thus, reduces cardiac output and in turn managed blood pressure. Metoprolol is known to have a negative inotropic effect, meaning it will reduce heart rate on intake (Morris & Dunham, 2021).

Answer 2

Metoprolol is an inotrope and is known to produce side effects including light-headedness and dizziness. This is because of falling cardiac output which may impair tissue supply to the brain. Most significantly, because of its negative inotropic action, it reduces the heart rate, so bradycardia is also a side-effect (Baleeqh Uddin et al., 2020).
Metoprolol causes sympathetic stimulation and can reduce the production of saliva in the mouth causing dryness. Metoprolol is known to cross the blood-brain barrier due to its lipophilic properties and can trigger sleeping difficulties and dream terrors. Additionally, a long term use of metoprolol also contributes to dyslipidaemia (Baleeqh Uddin et al., 2020).

Answer 3
Metoprolol being a negative inotrope acts by inhibiting the action of adrenaline on the beta cells in the cardiac muscles which consequently reduces the rate and strength of cardiac contractions (Katsi et al., 2019). The poor effect of adrenaline on the heart creates a synonymous parasympathetic condition, reducing heart rate and Blood pressure. Since metoprolol is reducing heart rate and Blood pressure by inhibition of adrenaline, it is caused as a negative inotrope (Katsi et al., 2019).
