Assignment Task

Introduction

This essay is about a patient I supported during his episode with anxiety and panic attacks during my previous placement. This essay supports service users with a holistic perspective, evidence-based care analysis, and clinical reasoning and decision-making (Benner, Hughes and Sutphen, 2019). The patient will be Jide as per NHS England and NHS Improvement's confidentiality policy (NHS England, 2019). Staff must know their duties to safeguard people, maintain confidential information, and secure data. Thus, 25-year-old Jide is single, depressed, and anxious (SCIE, 2019; HM Government, 2018).

A month ago, Jide went to the Accident and Emergency Department (A&E) for chest pain. The consultant found feelings of sadness and anxiety after the exam (Saini et al., 2020). Jide was transferred from A&E to the mental health community support team. The team assessed Jide at home for First Response Team (FRT) Assessment to create a care plan (The Royal College of Emergency Medicine , 2021). Even though he is aware that the team is able to assist, he is concerned about the health of his elderly parents. Taking Clozapine prescribed medication. Jide is insomniac despite eating and drinking well. He was instructed and encouraged to take his medication; his case was debated at the multidisciplinary team meeting; and the team sought anxiety and coping therapy for him.

Communication

Effective communication involves sharing thoughts, ideas, knowledge, information, viewpoints, and notions to attain a goal (Afriyie, 2020). To communicate effectively, both the speaker and the listener must comprehend one another (Center for Creative Leadership, 2023; British Heart Foundation, 2018).

According to Nasrollah, Rafii and Ghezeljeh, (2019), therapeutic support and care are necessary for the prevention of unfairness and discrimination in nursing. Vermeir et al. (2015) found that treatment chances can be wasted due to failed communication. NHS England, (2023); National Council of State Boards of Nursing, (2018) mandates nurses to evaluate, document, and report on all patient treatments. Jide's documentation should have all relevant information for consistency (England, 2017). Section 6 of the NMC Code requires practitioners to communicate with service users clearly, simply, and non-judgmentally (NMC, 2018).
