Case notes are an integral part of practice for many social workers. Research has shown that record- keeping practices have an impact on client outcomes, and that poorly formulated case notes can result in poor decision-making and adverse client outcomes (see Preston-Shoot 2003, Cumming et al. 2007). Throughout this course, you have learned about the importance of good record keeping about a client, in a way that is clear, impartial, accurate and complete. You have also been introduced to the ISBAR framework. ISBAR is used in a variety of circumstances in professional practice, including in clinical handover, report writing, case note writing, and email communication. ISBAR is an internationally recognised tool for communication in clinical contexts, allowing clinicians of any discipline or seniority to speak the same language (Hunter New England Health, 2009). 

Your task for these assessments is to write a case note and accompanying critical reflection. Your first case note/reflection will focus on your client meeting with Emma’s mother in week 8. Your second case note/reflection will focus on your interagency meetings in week 9. Each case note/reflection should be 1000 words in length (500 words for the case note; 500 words for the reflection). 

Your case notes should be clear and concise, and follow the ISBAR framework, using each element (introduction, situation, background, assessment and recommendation) to structure your writing. Your reflections should include critical discussion of your initial reactions to the case, and the factors that influenced the way you approached or interpreted the situation (e.g., emotional reactions, personal judgements, values and knowledge). You should also consider how your interpretation and approach changed or developed over time, and what influenced this process (e.g., hearing clients’ lived experiences, service provider perspectives, research, theory, social work skills and interventions, social work professional values and responsibilities).

  • Demonstrated capacity to structure communication using the ISBAR framework for effective communication.
  • Demonstrated ability for critical thinking and self-reflection about own interpretation and approach to the case scenario. 
  • Capacity to examine/identify the impact of relevant research, social work theories, values, ethical responsibilities, skills interventions on own interpretation and approach to the case scenario.
  • Professional writing, grammar, spelling, and presentation; and correct referencing style.
