Introduction: Through this Request for Proposals (RFP), the PHL Health Foundation (Foundation) will fund organizations to implement evidence-based strategies that promote and support public health especially one or more of the following: a) healthy eating, b) active living, c) reduction of sexually transmitted infections, d) air/land/water pollution or contamination and/or e) tobacco, alcohol or other drug use prevention. The Foundation is particularly interested in strategies that will improve the health of low-income populations that are disproportionately affected. This RFP represents one component of the Foundation’s efforts to address any of the above mentioned public health problems and is intended to support the work of organizations committed and well-positioned to improve the public’s health. To make greater progress on these critical health issues, this RFP will support implementation of evidence-based prevention strategies. For purposes of this RFP, “evidence-based strategies” refers to those recommended by leading health organizations (i.e., the Centers for Disease Control and Prevention, the Institute of Medicine, the Community Preventive Services Taskforce) or are otherwise supported by strong research and evaluation findings. The Foundation is particularly interested in supporting Healthy People 2020 objectives. Applicants will describe and substantiate the evidence base for their proposed strategies. Question: Write a grant using the information below that was adapted from the Kansas Health Foundation Healthy Living Grants - Request for Proposals (RFP). This assignment includes many of the aspects of public health leadership incorporated in this course. You will be graded according to the criteria listed in the RFP. You will have the opportunity to do related assignments prior to submission of the grant, e.g., writing SMART objectives, identifying community health partners, etc. Your grant writing team may choose to write a grant (real or for practice) to another entity. This is permissible but regardless of the requirements for the grant that your team chooses to write you must address the items highlighted in red text in this RFP for the purposes of meeting the requirements of this course. The topic we will do the grant proposal will be about "Tobacco, alcohol, or other drug use prevention" .
