The accident occurred at 16:30 on Mon 21st of March 2016 at Lake Hites NSW. 

Purpose

This report co-insides with the OHSMS manual that was done on the 10/1/2020. It had been reported that the tractor tipped over whilst doing a task. Also, emergency procedures around possible incidents wasn’t noted or was presented in previous meetings minutes.

Objectives

  • Present to management the findings and recommendations.
  • Making sure what has been put into practice is productive and successful. Allowing further improvement so it continues to do so.
  • To ensure commitment from management. This practice will show there will be accountability, leadership and direction.

Source findings

The possible root cause outcome of the tractor tipping over was:

  • Driving on the soil which was soft due to underground drain.
  • Tire on tractor may not have been correctly inflated when on the surface?
  • It was noted there was no adequate communication process between management and workers not to drive on soft soil. Especially on a sloped area.
  • Tractor was older which needed maintenance. A fault may have occurred when driving.
  • Workers didn’t have risk or hazard awareness training whilst out in the workplace.
  • It was presented that long work hours and no adequate breaks can contribute to fatigued conditions.
  • Working on the slopes was greater than the tractors recommendations.
  • Driver had a lapsed licensed.

   

Recommendations

  • Provide additional training to recognise risk/hazard awareness and report outcomes.
  • Ensure an effective communication process is present between workers and management regarding work-safety concerns in the workplace.
  • Consult with workers to enable feedback on work-related concerns. Once done train staff on what to do in case of emergencies may arise.
  • Ensure that the correct tractor is used for that environment.
  • If drivers license has lapsed, do not allow the worker to drive the tractor until the licence has been renewed.

All contract arrangements that have been made would need to include roles, responsibilities and what to expect when doing the job and safety compliance when working on a specific project.
Before starting to work in the field it would be recommended to introduce an induction discussion based on safety procedures and requirements.
Hold regular meetings as this will ensure everyone being informed. And, will allow workers to ask questions if a concern comes up whilst doing a job.
Ensure a clear communication channel is present. This way everyone knows whats going on and allows a feedback loop process.
