Tom, a 69-year-old retired geologist presents to his health care provider complaining of increasing shortness of breath especially on any exertion, and a chronic productive cough with wheezing on expiration. He states he is chronically fatigued and often wakes in the morning with a headache. He also is complaining of frequent episodes of heart palpitations. Tom has a 40-year smoking history but has recently quit smoking.  He has a history of chronic bronchitis.

and exhibits shortness of breath. She is anxious and in acute distress. Overthe past 3 weeks she reports frequent episodes of substernal chest pain
unrelieved by rest.  Previously, she only experienced chest pain with exertion and it was relieved by rest. She has a 30-pack year history of smoking and controlled mild hypertension.
