Specifically, you will undertake analysis of a real community data set, identify and discuss relevant social determinants of health, identify a vulnerable sub-group, explain how to engage with the community and how to undertake a more detailed needs assessment. Finally, you will explain how this information from a needs assessment could be used to inform health promotion planning in this community. Part A:  1. Create and populate a table a) Choose one of the communities in Appendix B – either Mt Isa OR Inala. b) Carefully analyse the Appendix B: Community Data for Analysis table for your chosen community and select three (3) categories of data that are relevant and linked, social determinants of health c) In the second column provide a summary statement that compares the local data to state and national average data, including percentages where relevant d) In the third column provide a brief statement (inference) as to how these differences may suggest health problems in the community. Part B:  Write an essay in which you address the following points: 1. Clearly introduce the task and outline topics covered in the essay 2. Based on the data you selected for Table 1 in Part A, discuss how each of these three social determinants impacts on health. 3. Based on your answer above at 2., make linkages between the social determinants/data to identify a vulnerable sub-group in the community that is likely to have health issues/unmet health needs). 4. Explain how as a community nurse you would engage with these vulnerable community members and how you would undertake a more detailed community needs assessment. 5.
