For this assessment you are required to have an actual feedback conversation (either formal or informal) with someone in your work environment during which you provide this person with feedback that you think would help them improve their work behaviour. 
After the feedback conversation, please answer the following seven questions:
1.What are the initials of this person?
2.What is your relationship (e.g., superior, colleague, subordinate) with this person?
3.For how long have you been working with this person?
4.Why did you think that this person is (potentially) problematic for your organization? In other words, what does this person do (or fail to do) and which problems does that cause? Or what problems may that cause in the future? For whom?
5.How did you prepare the feedback conversation?
6.Please describe the feedback conversation. What did you say? Did you say what you wanted and needed to say? How did you feel while having the feedback conversation? How did the other person react?
7.Please reflect back on questions 4 and 5. What did you learn from this feedback conversation? How does that relate to theory and research on feedback
