Five coach-loads of university students and their grandparents left UTS one warm summer morning on their way to Taronga Zoo for a day's outing. For lunch, they took cut sandwiches and cartons of rice pudding prepared at the university canteen. Before they had travelled far, many had started to eat their sandwiches and when nearing the zoo, the first victim began to feel ill. Soon other members of the group were unwell. When they arrived at the zoo the illness was acute and several people were taken to local hospitals.

Remnants of food and stool specimens from the patients who had been taken to hospital were collected. You are the acting medical officer of health in Sydney in charge of this investigation. Inquiries were made at the canteen where the sandwiches had been prepared. Those responsible for making them were questioned about the method of preparation and the source of the meats. The woman who prepared the sandwiches had taken some of the sandwiches home to share with her husband for supper. About two hours later they, too, became violently ill and were taken to hospital by ambulance.

The next step was to discover how these organisms had reached the food and why they had multiplied.

The meats were cooked at the food canteen. They were stored in a refrigerator and brought out day by day for the preparation of sandwiches.

There were two faults: the first that the worker used her hands freely to handle the meats and the sliced portions, and second that the refrigerator motor failed so that conditions were warm inside the cabinet. Conditions were ideal for multiplication – warm summer weather and lack of refrigeration.
